Retatrutide and vitamin deficiency: research overview
No retatrutide trial has measured vitamin status as an endpoint. What the research literature does establish is the environment in which micronutrient shortfalls develop: deeply suppressed appetite, reduced dietary variety, frequent gastrointestinal side effects and fast weight reduction — sustained for months or years.
The intake window
The 60-week semaglutide 2.4 mg RCT (Tronieri et al., AJCN 2026) is the cleanest quantification available in the class: ad libitum energy intake ran roughly 240–290 kcal/day below placebo for the entire treatment duration, not just during dose escalation. Every week in that window is a week in which dietary quality — not just quantity — decides whether micronutrient needs are met. Retatrutide, which produces larger average weight reductions, plausibly widens that window; its trials did not measure intake directly.
The mechanistic framework
Simancas-Racines and colleagues (Obesity Pillars 2026) mapped the chain explicitly: appetite suppression leads to reduced intake and dietary diversity; gastrointestinal intolerance compounds both losses and absorption; micronutrient vulnerability follows. Their review singles out thiamine and Wernicke risk where vomiting persists or intake falls markedly, and proposes a monitoring algorithm — while noting that most reported abnormalities remain subclinical or indirect and that the framework itself awaits formal evaluation.
What the societies advise
The four-society US advisory (Mozaffarian et al., 2025) and the European EASO/EFAD/ECPO consensus (Dobbie et al., 2026) converge on the same practical points: screen diet quality at baseline, manage gastrointestinal side effects actively, and monitor micronutrient risk during therapy. Neither recommends a specific vitamin supplementation protocol — and neither absolves a formulation from justifying whatever nutritional component it carries.
Why thiamine first
Among micronutrients, thiamine occupies a distinctive position in this setting: body reserves are limited, depletion can develop within weeks of inadequate intake, the historical rapid-weight-loss literature (bariatric surgery) shows exactly how the failure unfolds, and the consequences of missing it — Wernicke encephalopathy and Korsakoff syndrome — can be irreversible. That asymmetry is why Reta-B1 carries thiamine rather than a generic multivitamin gesture.
